What are the responsibilities and job description for the Case Manager - Youth Transitional Housing Program position at Arizona Youth Partnership?
ESSENTIAL FUNCTIONS:
- Collaborate with community shelter Directors, homeless providers, Mohave County Homeless Continuum of Care, and McKinney Vento liaisons to provide housing and related resources to 16-24 year old youth.
- Comply with contract deliverables and requirements including communications with project officers and attendance at contractors meetings as directed by the Program Manager.
- Recruit landlords willing to lease to youth in program.
- Maintain program operations and facilitate program development on-going to meet the needs of the participants (i.e.: housing, scheduling, resource referral, educational advancement, employment support, documentation maintenance, progress reports and review.)
- Collect data for HMIS (Homeless Management Information System) entry in clear, complete and legible manner
- Foster and facilitate welcoming and inclusive environment for Coordinated Entry, intake and drop-in services
- Ensure participants health, safety and welfare.
- Advocate for participants most appropriate education, employment and housing needs are met.
- Encourages participants to be involved in the community and fosters participant self-advocacy in activities using Positive Youth Development model.
- Participate in agency sponsored events (i.e.: outreach, networking and award ceremonies).
- Ensures compliance with all agency policy, procedures and standards.
- Attend and participate in regular meetings with project staff, contractors, and consultants to assure consistent understanding of the mission of the project, job responsibilities, program and funding compliance.
- All other duties assigned.
QUALIFICATIONS:
- Excellent planning, organization and time management skills that encourage a pro-active approach
- Bachelors degree in related field OR 2 years relevant, equivalent experience.
- Knowledge of rural communities and resources available to youth.
- Knowledge of Unaccompanied and Homeless Youth, Housing barriers, Prevention and/or Community Development theories.
- Knowledge of strategies supportive of Trauma Informed Care and Positive Youth Development models.
- Direct service experience and experience working with, and linking youth, to clinical services.
- Experience providing outreach education to local and state stakeholders.
- Excellent written and oral communication skills.
