Ark Healthcare and Rehab at St Camillus in Stamford is looking for a Director of Social Worker to join our Social Services department.
NURSING HOME EXPERIENCE IS REQUIRED. Must possess a BSW, MSW, or have qualifications to serve as a designee detailed in CT DPH §19-13-D8t Public Health Code.
Responsibilities include :
Develop social, psychosocial history, formulate care plan which addresses problems, needs, and concerns.
Participate in Interdisciplinary Meetings
Maintain compliance with MDS assessments, care plan review and revision.
Maintain compliance with federal and state documentation requirements .
Inform, educate, and advocate on behalf of patients and their resident rights.
Coordinate discharge planning, and assess patients for the potential of discharging.
Maintain compliance with required state assessment schedule, e.g. Assessment Pro Level I, IIs, Level of Cares and communicate changes or pertinent information to IDT.
Serve as point of contact with hospice as well communal agencies and care providers for post-discharge environments.
State / Federal Survey experience
Participate in Quality Assurance and Performance Improvement Initiatives (QAPI) Program.
Communicate with Probate Court in matters related to Conservatorships
Duties as assigned.
Must possess strong work ethic, communication skills, and writing skills.
