Responsibilities
- Provider individualized client-centered services to individuals enrolled in the Mental Health Rehabilitation Program.
- Complete initial screening of all prospective clients
- Administer and score LOCUS/CALOCUS (must be an Approved Clinical Evaluator)
- Ensure that client is assessment per MCO guidelines upon entry into program and then reassessed every 120 days.
- Regularly assesses consumers goals and strengths as well as needs for community resources.
- Provide crisis interventions services as needed and in accordance with agency policies.
- Ensure that each consumer has an Individualized Recovery Plan on file within 30 days
- and is updated at least every 120 days.
- Insures that all ISRPs are signed by the LMHP, consumer and psychiatrist.
- Collaborates with Medical Director to assess and develop social diagnosis, plan for treatment and continuity of care needs.
- Ensure that services rendered by the MHP and MHS are in accordance with the consumers ISRP.
- Ensure appropriate discharge and transitional planning of all consumers.
- Conducts monthly trainings, staff meetings and/or individualized supervisions with MHP/MHS team members.
- Communicates effectively and works cooperatively with employees, families, schools, individuals, and the public.
- Documents services provided in a concise and timely manner in accordance with agency standards.
- Directly involved in agency marketing and community outreach activities.
- Participates in all scheduled staff meetings, trainings and supervisions.
- Adheres to and upholds the ethical standards of the profession.
- Performs additional responsibilities as needed and/or outlined by the Program Director.
- Meets or exceeds productivity expectations.
- May collaborate with other professionals, but primarily works independently.
Minimum Qualification:
- A Master of Social Work, Counseling, Psychology or Sociology in a mental health related field
- Licensed Clinical Social Work or Licensed Professional Counselor
- Unrestricted state issued license
- Professional liability insurance
Preferred Qualifications
- Has previous Mental Health Rehabilitation experience
Special Requirements (must have all of the following):
- Must have a valid driver’s license, good driving record, and a reliable vehicle
- Must have the ability to obtain clearances as defined by regulations.
