Greetings! We hope you're having a great year so far. We're now hiring for Bilingual Patient Support Case Manager. This State of Georgia government job is an excellent opportunity for a skilled professional to act as a primary point of contact for patients, coordinating their care and providing support by facilitating communication between them and healthcare providers, while being fluent in both English and Spanish to effectively serve diverse patient populations. Responsibilities include assessing patient needs, developing care plans, navigating complex healthcare systems, and advocating for patients' best interests, all while considering cultural sensitivities. Job Duties and Responsibilities : Regular and direct communication (maybe face to face, via telephone, or in writing) with victims of violent crimes (i.e., applicants), family members of the victims, law enforcement, district attorney offices, social service provides, healthcare practitioners, etc., to gather information to use in the claims process High level of integrity and ethics and the ability to work with information of a sensitive nature requiring strict confidentiality.
- Strong Listening skills to address to understand and provide guidance and instruction as required
- Coordinate and act as a liaison between the applicants (i.e., victims), staff members, other advocates, service providers etc., that is timely, effective, efficient, equitable, and client-centered
- Possess and demonstrate Analytical skills along with thoroughness to ensure applicants obtain the best options and outcomes afforded through the Crime Victims Compensation program
- Provide critical information, referrals, assistance and follow-up to applicants, professionals within the criminal justice system (lawyers, District Attorneys, etc.,) or social service systems with crime victim compensation claims.
- Develop effective working relations throughout the entire case management process Required Competencies or Experiences : Effective communication skills, Excellent organizational and time management skills, Problem solving skills and ability to multi-task
- Compassionate with teamwork skill
- Preferred Qualifications : One or more years of experience as a social service provider, a case manager, within human services, working with programs related to Victim advocacy, victim’s witness assistance, domestic violence, sexual assault, victim’s compensation, vocation rehabilitation or related field Entry Qualifications Associates Degree in a related field (human services, social services, vocational rehabilitation, from an accredited college or university. Note an equivalent combination of education and job specific experience that provided the knowledge, experience and competencies required to successfully perform the on at the level listed.
