Job Summary
The Registered Nurse in Home and Community Based Services delivers high-quality care to patients in their homes, as prescribed by physicians or authorized by state/program regulations. This role coordinates care with other healthcare professionals, ensuring continuity of patient care through effective planning and communication.
Responsibilities
- Provide clinical services within scope of practice, adhering to infection control standards and plan of care/service plan requirements.
- Conduct comprehensive clinical assessments in accordance with federal and/or state program requirements.
- Verify patient eligibility and medical necessity for services as defined by payer source and agency policy.
- Develop and revise individualized plans of care/service plans according to federal and/or state program requirements.
- Ensure plan of care frequency and duration meets patient needs, initiating revisions as needed with physician approval.

Requirements
- A minimum of one year experience as an RN is preferred.
Licenses/Certifications
- Current RN licensure in state of practice is required.
