Responsibilities
- Coordinate patient treatment plans by collaborating with healthcare providers and support staff.
- Assist in medication administration and ensure compliance with HIPAA regulations.
- Manage discharge planning for patients, ensuring they receive appropriate follow-up care.
- Maintain accurate patient records and documentation in accordance with medical office standards.
- Conduct physical examinations as required and assist in triaging patients in urgent care settings.
- Serve as a liaison between patients, families, and healthcare professionals to facilitate effective communication.
- Stay informed about home community care resources to provide patients with comprehensive support.
- Uphold high standards of patient care while navigating through various healthcare environments, including Level I Trauma Centers.
Qualifications
- LPN License
- Previous experience in a medical office or healthcare setting is preferred.
- Familiarity with home community care practices is a plus.
- Knowledge of medication administration protocols and medical terminology is essential.
- Experience in discharge planning and working within nursing home environments is advantageous.
- Ability to work collaboratively within a multidisciplinary team while maintaining patient confidentiality.
- Strong organizational skills with attention to detail to manage multiple tasks effectively.
