Full-time position with required travel
Compensation: $33,000 - $38,000 a year
Job summary: The Transition Specialist (TS) will work with the Vocational Services Manager to develop and implement a program focused on activities for youth ages 16 to 21, geared to prepare for life after high school. Work will include the development and implementation of a course of study focused on work readiness, self-advocacy, job exploration, post-secondary education, and independent living skills.

What You’ll do:
- Evaluation of various curricula encompassing self-advocacy, job exploration, post-secondary education, and independent living skills to young persons with disabilities, Deciding with Vocational Services Manager which curriculum shows the most promise for youth of transition age.
- Working with the Vocational Services Manager to devise methods of recruitment of youth to participate in the chosen curricula. In addition, providing information regarding the transition program to schools, parents, and potential participants.
- Collaborating with teachers of youth with disabilities in focusing on Individualized Education Program (IEP) goals, transition plans, and working on these goals outside of the classroom.
- Coordinating activities of the program with staff of SWEP and Kids Career Club.
- Identifying potential community sites for implantation of the transition program and promoting the program to the organizations and individuals at these sites. Forging necessary partnerships with these individuals and organizations to implement the program.
- Working with VS Manager and ARS to devise method of data collection regarding participation in the program; data should document length and duration of participation, level of success with curricula, and activities carried out in the various community settings.
- Evaluating the outcomes and satisfaction with the transition program for both youth and their families and reporting these results to community partners and to grant project officer. Modification of the program will be based on evaluation feedback.
- Functioning as part of the Vocational Services team and attending all staff meetings and training required by the VS Manager, CEO, ASN Board, and CPRC Grantors.
* Travel throughout the community and statewide is required.
What You’ll need:
- Bachelor’s degree.
- Valid driver’s license and access to a working vehicle.
- Previous experience working in special education or with individuals with disabilities.
- Ability to work independently and as part of a team.
- Must be able to meet the following physical demands: close vision and manual dexterity. Required to stand, walk, reach with arms and hands, climb or balance, stoop, kneel, crouch, or crawl.
