Job Summary:
- Provide psychosocial services and case management functions throughout the continuum of care.
- Perform healthcare insurance notifications and reviews as needed.
- Maintain performance improvement activities within the department and participate in PI initiatives.
- Built strong relationships with external agencies and internal departments/personnel.
- Optimize patient/resident safety and reduce errors in job function environment.
- Communicate with healthcare team, patients, and family regarding patient needs.
- Maintain knowledge of community resources and be sensitive to illnesses/injuries that impact patient recovery.
- Participate in facility meetings and facilitate patient/family meetings as needed.
- Support patients and families through the grieving process.
- Treat patients and families with respect and dignity.
Skill/Education Requirements:
- High School Diploma Required
- Graduate of social work curricula or related human services field, LPN preferred
- 2 years of case management experience preferred
