Job Summary
1. Demonstrates excellent patient relations skills on the phone and in-person, e.g. smiles, cheerfully greet
patients, engages all patients in a genuine, warm and caring manner.
2. Ensure complete and accurate patient demographics and current insurance information.
3. Processes patient referrals to specialty providers by preparing medical charts and records for referral
patient visits and providing appropriate clinical information to specialist.
4. Obtain authorizations for patient referrals to specialists, diagnostic testing and procedures. Advise
physician if authorization request is denied.
5. Reviews details and expectations about the referral with patients.
6. Maintain ongoing tracking and appropriate documentation on referrals by recording all referrals into
referral log book, or EHR documentation if available.
7. Acts as a resource for patients or providers who have questions or problems relating to referrals.
8. Ensure that referrals are addressed in a timely manner, prioritizing urgent items appropriately.
9. Identify and utilize cultural, language and community resources. Establish and maintain relationships
with identified service providers.
10. When necessary, assist back-office personnel by providing MA duties (if applicable)
11. When necessary, assist front office personnel by provide office assistant duties (if applicable).
12. Maintains regular consistent attendance, personal appearance, punctuality, and adherence to applicable
health and safety procedures
13. Performs other work-related duties as assigned.

Requirements/Skills
- High School Diploma required or equivalent
- Thorough understanding of Medicare, Medicaid, HMO’s, PPO’s and private insurance companies.
- Data Entry Skills
- Basic computer knowledge
