- Seeking a BE / BC Hospice and Palliative Medicine physician for full time position.
- Work as a collaborative member of an interdisciplinary team to provide Palliative Medicine consultation in the hospital and clinic settings.
- Provide inpatient and outpatient Palliative Medicine consultation services.
- Consult with the attending physician and the Interdisciplinary Team to contribute to the patients plan of care.
- Facilitate clarification of patient and family goals of care.
- Provide expert physical and non-physical symptom assessment and management.
- Facilitate access to appropriate supportive care services.
- Participate weekly in interdisciplinary team meetings to review active cases.
- Appropriately document patient care services to support professional billing for these services.
- Serve as collaborating physician for Palliative and Supportive Care Service nurse practitioners.
- Collaborate with Hospice Medical Director to help provide mutual medical coverage in case of eithers absence.
- Participate in Quality Improvement Initiatives through the Center of Palliative Medicine.
- Assist in the development of standards of care through the Center of Palliative Medicine.
- Provide Palliative Care education for staff, colleagues, students, and community.
- Perform other duties as requested.
- Adhere to the practice of confidentiality regarding patients, families, staff, and the organization.
- At this time, no after-hours Palliative Medicine on-call coverage is expected, though with service growth this could be a development in the future.
