POSITION DESCRIPTION
Case Manager of Family Outreach (Fort Worth)
Reports To: Director of Family Outreach Center and Executive Director Family Outreach Services
FLSA Status: Exempt
ARMS OF HOPE MISSION & VISION STATEMENT:
To transform the lives of disadvantaged children, youth and families by providing hope and support in a nurturing, Christian environment.
Provide support to families through AOH Family Centers in the inner-city areas of Denton & Fort Worth, San Antonio, Houston and Dallas. Help families in any needed area of physical, emotional or spiritual support. Provides referrals to non-profits, facilitates and helps with Medicaid acceptance, organizes Christian counseling, partners with local entities/churches to ensure success, provide follow-up care for former residents of AOH that live in the area.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Coordinates and Develops Partnerships within the Denton, Fort Worth, Houston, San Antonio and Dallas Community Area to meet clients needs.
Composes and types emails and all needed communication.
Collect, reviews and follows through with assigned applications for physical, emotional and spiritual needs.
Work in close conjunction with partner AOH entities.
Provide referrals to other non-profits in the area.
Assist families in qualifying for Medicaid and other government funding.
Organizes and oversees Christian counseling for families which includes Assessment, Diagnosis and Treatment.
Attend staff meetings, trainings, and in-services as needed.
Provide follow-up care for past AOH residents thru phone calls, emails and face-to face visits.
Work in close collaboration with other case workers on AOH campuses to ensure maximum efficiency for referrals into our residential programs.
Provide Crisis Management and Family Management Services including life skills such as budgeting.
Assist clients in becoming knowledgeable in vocational and educational skills.
Coordinate a volunteer/intern base within the community and local church
Other duties as assigned
QUALIFICATIONS
Bachelor’s degree in Social Work/Psychology/Ministry or a related field or two to four years related experience and/or training; or equivalent combination of education and experience required. Master’s Degree and some experience in Social Work preferred.
CERTIFICATES, LICENSES AND REGISTRATIONS: Current Driver's License Required. Other licenses (Social Work, counseling, etc.) preferred.

Pay: $36,000.00 - $40,000.00 per year
