Location : Arnold Walter Nursing and Rehabilitation Center
Shift : Full-Time | 8 : 00 AM – 4 : 00 PM

Key Responsibilities :
- Assist in developing, implementing, and evaluating social service programs for residents.
- Maintain knowledge of federal and state regulations, recommending policy changes as needed.
- Participate in discharge planning, resident assessments, and social care plan development.
- Interview residents and families to gather social histories and develop individualized care plans.
- Coordinate transportation and connect residents with community resources as necessary.
- Educate residents and families on Medicare, Medicaid, and other financial assistance options.
- Collaborate with medical staff and interdisciplinary teams to address residents’ needs.
- Maintain detailed and informative progress notes.
- Support quality assurance initiatives and assist during government surveys and inspections.
- Participate in committee meetings, including Infection Control, Policy Advisory, and Quality Assessment.
- Ensure residents' end-of-life care decisions (MOLST / POLST) are properly documented and implemented.
