POSITION SUMMARY:

Processes and ensures the accuracy, adequacy and completeness of information by analysis and is responsible for handling patient information for any purpose.

REPORTS TO: Director of Health Information Management

DUTIES AND RESPONSIBILITIES:

** 1. Must be adept at hospital electronic systems as they pertain to Medical Records and electronic verification of patient information.

** 2. Maintains logs and updated material for downtime procedures.

** 3. Ensures the accuracy, adequacy and completeness of information by analysis, identification of deficiencies and subsequent assignments to appropriate personnel for corrections

** 4. Pulls/delivers inpatient and outpatient previous admission records to create a unit record. Makes additional volumes as necessary. Maintains MPI logs as required.

** 5. Verifies and corrects MR numbers, episodes, history information, etc. Corrects and updates demo recall.

** 6. Analyzes records for deficiency identification and assign to responsible personnel for completion.

** 7. Maintains physician delinquency control. Issues warnings and suspension letters as necessary.

** 8. Maintains incomplete record control. Generates weekly reminder letters listing
incomplete charts.

** 9. Retrieves and files medical records.

** 10. Performs final checking procedures.

** 11. Maintains a working knowledge of legal requirements for Release of Medical Record information. Appropriately handles lawyer requests, subpoenas and provides records to court.

** 12. Processes correspondence and requests for medical record information

** 13. Provides Record Review and maintains on-going review log.

** 14. Maintains confidentiality of patient/department/hospital information.

** 15. Provides departmental reception and answers telephone.

** 16. Maintains software indices and registries (Donor Recovery and Occupational Lung) as required.

** 17. Assembles records into a standardized chronological format according to the type of service (i.e. Inpatient, Skilled Nursing Facility charts, Ambulatory Surgery, Short Stay Observation, Emergency Services).

** 18. Performs chart analysis for Inpatient and SP,ED records and Clinical Pertinence Reviews.

** 19. Understands and demonstrates the importance of satisfying the needs of the customer/patient by interacting with him/her in a friendly and caring way, being attentive to the customers needs, both psychologically and physically, and by taking the initiative to maintain communication with the customer in order to provide and secure a pleasant experience at the facility.

** 20. Maintains knowledge of all regulatory agencies.

** 21. Maintains professional skills and refines expertise through appropriate educational/development activities and professional affiliations.

22. Has working knowledge of Corporate Compliance regulations and promptly reports any potential Corporate Compliance violations to the Corporate Compliance Officer.

23. Demonstrates safety in work habits and personal conduct.

24. Is familiar with and demonstrates Continuous Quality Improvement principles and participates in the process as appropriate, focusing on meeting the customer service needs of the internal and external customers.

25. Must possess the highest ethical standards with respect to discretion and regard for confidential information.

26. Is responsible for attending all annual mandatory educational programs as required by position.

27. In order to provide the highest quality care to our patients and residents, individuals may be required to work hours beyond those normally scheduled. Work schedules may be subject to change based on the needs of the department.

EDUCATION/TRAINING:

College education or equivalent. Completion of Health Information Technology program preferred.

EXPERIENCE:

Four years of hospital experience in the Health Information Management Department required. General knowledge of medical, hospital, or other health related office operations helpful. Knowledge of medical terminology preferred.

PHYSICAL/MENTAL DEMANDS:

Intermittent sitting, walking and standing are required. Work requires visual acuity to use computer terminal and to verify accuracy of written material. Operation of standard office equipment required.

EXPOSURE CATEGORY:

Category III. Tasks that do not involve exposure to blood, body fluids or tissues. Category I tasks are not a condition of employment.
