- On-call responsibilities
- Identify patients requiring Social Work services through high-risk screening tools.
- Assess patient needs, issues, and available resources to support comprehensive discharge planning.
- Develop and implement creative solutions to complex care issues in collaboration with the healthcare team.
- Provide emotional support and education to patients and families, promoting informed decision-making.
- Serve as a liaison between the hospital and community agencies, ensuring continuity of care.
- Coordinate patient discharges and transfers, including arranging home health services, rehabilitation, and necessary equipment.
- Lead and participate in multidisciplinary meetings, patient care rounds, and hospital-wide education initiatives.
- Maintain accurate and timely documentation in accordance with departmental standards.
- Provide crisis intervention services, including suicide risk assessments as needed.
- Support educational opportunities for student field placements and professional development.
- Education: Master’s Degree in Social Work from an accredited (CSWE) school. Must possess NYS licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
- Experience: Hospital-based social work experience highly preferred.
- Skills: Strong communication, collaboration, and problem-solving abilities. Ability to navigate complex care planning and advocate for patient needs.
- Personal Qualities: Compassionate, patient-focused, and committed to interdisciplinary teamwork.
- Some physical effort required, including light lifting, reaching, and stooping.
- Frequent standing and walking throughout work hours.
- Category II: Potential exposure to blood, body fluids, or tissues, with occasional unplanned Category I tasks.
