Job Title: Integrated Case Manager
Department: Inpatient Case Management
Job Type: 13-Week Contract (Possible Extension)
Location: Detroit, MI 48202
Shift: 8:30 AM - 5:00 PM (rotating schedule, weekend availability required)
Job Summary:
The Integrated Case Manager for Population Health collaborates with patients, families, and care teams to assess, plan, coordinate, and advocate for comprehensive care. This role addresses complex patient needs, promoting safety, quality, and cost-effective outcomes across care settings.
Key Responsibilities:
- Conduct comprehensive patient assessments.
- Develop personalized, patient-centered care plans.
- Advocate for and engage patients and families in their care.
- Coordinate transitions between care settings for seamless management.
- Address end-of-life care and advanced planning needs.
- Facilitate interprofessional care conferences for complex cases.
- Evaluate and adjust care plans based on patient outcomes.
- Support process improvement initiatives and risk reduction.
- Address cases involving abuse or violence as required.
Required Skills:
- Strong communication, interpersonal, and customer service skills.
- Expertise in care coordination, discharge planning, and case management principles.
- Knowledge of Medicare/Medicaid regulations and social determinants of health.
- Ability to manage multiple priorities effectively.
- Compassionate with strong advocacy and problem-solving skills.
Preferred Experience:
- Background in home health, community health, or managed care.
- Familiarity with preventive service guidelines and behavior change theories.
Job Type: Contract
Pay: $45.00 - $55.00 per hour
Expected hours: 40 per week
