Position Description:
The Case Manager is responsible for working with primary counselors to ensure treatment plan is updated to include case management services. Handling much of the client administrative work including but not limited to delinquent patient accounts, submitting and following up on MA applications, and serving as communication coordinator between internal staff and external providers. The case manager will adhere to the vision and mission of the company.

Essential Functions:
- Responsible for review and follow up of delinquent patient accounts
- Monitors the overdue balance report on a daily basis
- Meets with patients on a weekly basis (or more if necessary)
- Documentation of interventions in preparation for financial discharges
- Weekly meeting with ED and DOF to review noncompliance
- Coordinates with other insurances or funding sources as necessary (Veterans Administration, Drug and Alcohol Commission, etc.
- Obtain consents for all referral sources and existing supports which will include diagnosis and treatment plan for health providers when possible
- Documentation of in-person meetings, phone calls, referrals, support, planning and follow up
Qualifications:
- Bachelor’s degree.
- Needs to be highly organized
- Detail oriented
- Extremely approachable and customer service oriented
- Ability to develop contacts and network (good communication skills)
- Healthcare account management or case management experience would be a plus
- Must be available to work at least 1 weekday starting at 4:30am to provide an opportunity to meet with early dosers.
