Direct Support Professional (Weekend Only)
Job Description
We are seeking a compassionate and dedicated Direct Support Professional (weekend only) to join our growing team. As a Direct Support Professional, you will play a vital role in providing high-quality support to individuals with disabilities or seniors in the community, home settings, group homes or nursing homes. The ideal candidate will be able to work with individuals who require assistance with daily living activities and will provide a safe, supportive and nurturing environment. Responsibilities:
- Supports the development and implementation of person centered plans.
- Reviews, understands, and implements the ISP (Individual Support Plan) of each individual supported.
- Provides support as outlined and directed in the ISP Plan.
- Provides assistance and/or instruction with ADLs and IADLs as required, while promoting independence in these areas of growth.
- Attends in-service training sessions as required.
- Completes annual trainings as required.
- Encourages independence at all times, whenever possible and safe.
- Assists with transfers and/or ambulation as required and within your limitations.
- Practices universal safety precautions regarding infectious disease.
- Documents and maintains accurate records for each individual’s progress toward outcomes, daily activities, and care requirements for each shift worked.
- Participates in annual ISP meetings and/or multidisciplinary team meetings as required.
- Ensures open lines of communication between individuals, families, and the agency.
- Acts professionally at all times.
- Maintains current employment records by informing the director of human resources any demographic changes, trainings completed and updated auto and auto insurance requirements.
- Informs supervisor of any changes to an individual's care needs or desires to change outcomes.
- Reports incidents in a timely manner as directed by the Incident Management Policy.
- Complies with all federal, state, county and agency regulations as they relate to the provision of waiver services.
- Performs other duties as determined necessary.
Qualifications:
- Experience working with individuals with disabilities or seniors in a community setting, home care setting, group home or nursing home.
- Familiarity with patient care and vital sign monitoring.
- Willingness to learn and adapt to changing situations and client needs.
- Ability to maintain confidentiality and adhere to HIPAA regulations.
- High school diploma or equivalent required, associate or bachelors degree in a related field preferred.
- Valid drivers license and reliable transportation to and from work.
- Ability to lift, bend and move clients as needed.
- Ability to work varied shifts, including evenings, weekends and holidays.
Skills:
- Patient observation and patient care.
- Experience working in a community setting, home care setting, group home or senior nursing home facility.
- Familiarity with patient care and vital sign monitoring.
- Familiarity with first aid and basic medical procedure.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Ability to lift, bend and move clients as needed.
