Overall Responsibility
Assess client financial needs and assist with meeting those needs by completing Medicaid and Marketplace applications along with sliding fee application.
Key Tasks and Responsibilities
Key Tasks and Responsibilities for Navigators Only
- Meeting with clients in person for financial assessment
- Obtaining client demographic information to complete new charts within the Electronic Health Record system
- Complete a financial assessment with all clients including sliding fee applications
- Schedule appointments for all new clients
- Coordinate initial behavioral health appointments in accordance with Open Access policies
- Update and secure all financial assistance programs as directed
- Assist client’s with Medicaid applications including
- Presumptive Eligibility
- Aged, Blind, and Disabled
- Adult Expansion Group
- Former Foster Youth
- Families
- Assist clients with complex Medicaid cases by providing authorized representation to Family Support Division
- Assist clients with Marketplace applications.
- Returning phone calls from clients when requested
- Coordinate with providers in multiple departments for continuity of care
- Attend and run booths during outreach events
- Attend yearly Centers of Medicare & Medicaid conference as required by Centers of Medicare & Medicaid Certified Application Counselor and Navigator Designated Organization agreement
- Keep protected patient health information confidential
- Ensure weekly report is submitted on time per Centers of Medicare & Medicaid reporting regulations
- Attend required weekly Navigator meetings per Certified Application Counselor Designated Organization agreement
- Complete all weekly MAC assignments
- All other duties as assigned

Degree Requirement
High School diploma required, Bachelor’s Degree in Social Work preferred
Licensure Requirements
Must obtain Certified Application Counselor license within 30 days of hire
Experience Requirements
Must have at least two years of clinical office setting experience. Experience with Medicaid and Marketplace preferred but not required.
