Key Responsibilities:

- Collaborate with the treatment team to provide comprehensive mental health and substance use recovery-oriented services, ensuring continuity of care and addressing the unique needs of each client.
- Develop and implement effective discharge plans, working closely with clients, family members, and significant others to ensure a smooth transition to aftercare.
- Conduct thorough assessments of clients' psychosocial situations, identifying areas of strength and need, and developing personalized treatment goals and interventions to promote optimal functioning.
