Asbury Health Center is seeking an experienced MDS Coordinator to join our team. The MDS Coordinator will be responsible for coordinating and overseeing the completion of resident assessments and the development of care plans in accordance with federal and state regulations.
Responsibilities:
- Collaborate with interdisciplinary team members to ensure comprehensive resident assessments are completed in a timely manner
- Develop and implement individualized care plans based on resident needs
- Monitor and update care plans as necessary
- Ensure compliance with federal and state regulations related to resident assessments and care planning
- Provide education and training to staff on MDS and care planning processes
Qualifications:
- Current RN license
- Minimum of 2 years of experience as an MDS Coordinator
- Knowledge of federal and state regulations related to resident assessments and care planning
- Strong communication and interpersonal skills
- Ability to work collaboratively with interdisciplinary team members
