The Chaplain will be responsible for, but not limited to:
- Provides pastoral or spiritual counseling in keeping with patient/family beliefs, and in accordance with Ascend Hospice & Palliative Care MA policies and procedures.
- Completes spiritual/pastoral assessment as indicated and appropriate, completed within 1 day of admission.
- Develops an individualized spiritual/pastoral plan of care, according to regulations.
- Offers patients and families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feeling, beliefs and values.
- Responds to hospice patient/family needs as requested.
- Coordinates and integrates pastoral services with other patient/family care services in order to ensure a continuum of care, including continuity of care following death or discharge.
- Prays with families and patients, if and when desired by the patient and/or family unit.
- Effectively participates as a member of the multidisciplinary Hospice team, providing quality Hospice care to the patient and his/her family.
- Provides bereavement services, in accordance with Ascend Hospice & Palliative Care MA policies and procedures. Initiates bereavement assessment between 7 and 14 days after the death of the patient.
- Documents a plan of care for the bereaved survivors, based on the IDT's assessment.

Education:
Master's Degree in Divinity
Professional Experience:
- 2-4 years of Chaplain experience with end of life care
- At least 1 Unit of CPE completed
- Ability to provide non-denominational spiritual care
- Excellent written and verbal communication skills
