Role Description
This is a Per Diem role for a Licensed Clinical Social Worker at Ascend Home Health located in Rochelle Park, NJ. The Licensed Clinical Social Worker will be responsible for providing medical social work, case management, mental health, and behavioral health services to our home health clients in both Bergen and Essex counties.
Qualifications
- Social Work and Medical Social Work skills
- Case Management skills
- Mental Health and Behavioral Health skills
- Experience in providing social work services in healthcare settings
- Strong communication and interpersonal skills
- Ability to work effectively in a team and independently
- LCSW license in the state of New Jersey
- Master's degree in Social Work
