What are the responsibilities and job description for the Bereavement Supervisor position at Ascend Hospice & Palliative Care KS?
Now Hiring - Bereavement Supervisor
Hours:
The Bereavement Supervisor, will be responsible for but not limited to:
- Maintain and file all Bereavement paperwork according to Ascend Hospice Protocol
- Ensures compliance with care plan contacts for bereaved according to grief risk and contact frequency
- Maintain active list of State Support groups
- Prepares mailing labels and letters for bereavement mailings to bereaved family and/or friends in the scheduled time frame (5 months, 12 month anniversary cards, and 13 month bereavement letters to bereaved)
- Refers family and fiend to assigned chaplain for further assistance
- Follows verbal and written instructions accurately and in a timely manner
- Creates bereavement reports from HPMS system
- Audits to maintain compliance
- Places 2-3 month follow up calls to low risk bereaved
- Completes documentation of bereavement calls according to compliance guidelines
- Performs copying and routine coping as requested
- Distributes mail and faxes to appropriate personnel
- Sends outgoing mail and faxes as requested
- Keeps files, records and supplies well organized
