Now Hiring - Intake Coordinator
The Intake Coordinator is responsible for, but not limited to:
- Completes initial inquiry forms for all potential admissions, and forwards completed forms to the appropriate Hospice Nurse and Director of Clinical Services and/or Administrator
- Insures that whenever possible, all referrals are converted to admissions
- If referrals do not convert to admissions, the Administrator will be notified
- Track all inquiries and referrals daily. This report will be given to the Administrator weekly
- Assists patients, families and responsible parties in completing the admissions packet as needed
- Works collaboratively with appropriate staff to insure written or verbal consents are obtained

Education:
Associates Degree in a related field
Professional Experience:
- Preferably 5-8 years
- Experience in a related capacity
- Must have a Valid Driver's License and reliable source of transportation
- Must be willing to travel
- Excellent communication, interpersonal and organizational skills
