Job Overview
AscendCare Home Health Solutions LLC is seeking compassionate and dependable Caregivers/Home Health Aides (HHAs) to join our team and provide high-quality care to clients in their homes. Our caregivers play a vital role in helping individuals maintain dignity and independence while receiving supportive services in the comfort of their familiar surroundings.
As a Home Health Aide, you will assist with personal care, light housekeeping, and companionship to clients in need, under the supervision of a Registered Nurse.
Key Responsibilities
- Provide personal care assistance including bathing, grooming, dressing, and toileting
- Assist with ambulation, transfers, and mobility support
- Medication administration, reminder, assistance
- Prepare and serve meals; assist with feeding as needed
- Perform light housekeeping tasks such as laundry, dishes, and tidying
- Offer emotional support and companionship
- Monitor and report changes in client condition to the RN supervisor
- Follow individualized care plans and document services provided
- Transportation, errands, shopping
Qualifications:
- High school diploma or GED Completion of a Home Health Aide, CNA, or PCA training program
- Must be at least 18 years of age
- Ability to pass a criminal background check and TB screening
- Reliable transportation
- Strong communication and interpersonal skills
- Compassionate, patient, and dependable
- Willingness to learn
- Respectful of clients, client's homes, and their wishes/rights
Preferred Qualifications:
- Previous home care experience
- Experience working with seniors or individuals with disabilities
- Culturally sensitive and respectful of client preferences
- Familiarity with dementia care
- CPR and First Aide Certified (will train)
- Experience with medication: administration, reminders, and assistance
- Experience documenting in Electronic Health Record System (Ex: Point Click Care, Well Sky, Eldermark)
Schedule & Work Type:
- Part-time and PRN (as needed) shifts
