Our client is looking for passionate and compassionate individuals to join their team as Social Services Coordinator , dedicated to helping all members of the homeless community, including Veterans and their families. This role is crucial in connecting individuals to the Supportive Services for Veteran Families (SSVF) program and various community resources.
Position Summary : The Social Services Coordinator will be the first point of contact for individuals experiencing homelessness or at imminent risk of becoming homeless. They will assess eligibility for support services and provide necessary referrals. This position involves managing a caseload of 20-30 clients at a time, allowing for meaningful one-on-one interactions as they guide individuals through the qualification process.
Key Responsibilities :
- Build rapport with individuals and families experiencing homelessness and assess their eligibility for various programs.
- Generate referrals to the SSVF program and other community resources.
- Coordinate emergency services for clients who are literally homeless.
- Assist clients in obtaining necessary documentation and provide transportation as needed..
- Maintain accurate records in the Homeless Management Information System (HMIS) and other databases.
- Work a flexible schedule to meet client needs and support agency goals.
Position Requirements :
On site : Mon- Fri 8am-5pm
