General Job Description
The DePaul Community Health Centers Health Coach will interact with patients in a variety of ways creating an environment that lends to positive, motivational counseling sessions. In this supportive role, the health coach will use a variety of tools to assist the patient in maintaining a healthy lifestyle as prescribed by the health care provider. This is a full time position and the candidate will work on site.
Duties & Responsibilities
- The Health Coach collaborates with the patient to identify feasible action strategies that could be implemented toward reaching personal goals. The expected outcome is to achieve greater control of the patient’s chronic disease.
- Manages a schedule and caseload of patients by providing disease management and coaching services.
- Documents patient progress using the Care Plan in ECW to track and monitor the progress towards measurable goals.
- Uses appropriate pre-approved educational tools and action plans.
- Responsible for accurately recording pertinent patient data for the purposes of detailed record keeping and tracking.
- Implements a range of interventions/skills including counseling, motivational interviewing, coaching, education, referral and advocacy to gain access to services and resources for patients.
- Responds to and documents clinical calls on a daily basis.
- Communicates with the provider and healthcare team regarding the patients’ progress.
Requirements
Bachelor’s degree or higher in health promotion, kinesiology, counseling, or related health field is preferred.
Five years clinical experience in lieu of bachelor's degree. Coaching certification from a nationally recognized coaching organization preferred.
The candidate must obtain nationally recognized certification within nine months of employment as a Health Coach.
Preferred Experience
Nursing, Social Worker, Health or Life coaching experience are a plus.
Medical Assistant with a minimum of five years experience in a primary care setting may be substituted.
