Lakeshore at Siena, part of Ascension Living is looking for an experienced, caring and compassionate Social Worker to join our team. This beautiful 96-bed skilled nursing community is located on Wisconsin’s beautiful Lake Michigan coastline, where you can enjoy scenic views. When additional therapy is needed after surgery, illness or hospital stay, we provide rehabilitation and short-term care.

Investigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems.
- Interview patients, assess priorities and document case activity.
- Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
- Assist clients/families with obtaining community assistance by referral to proper resources.
- Develop and maintain working relationships with community health, welfare and social agencies and seek creative means to assist the client with his/her needs.
- Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.
Licensure / Certification / Registration:
- Master Social Worker, preferred.
Education:
- High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.
Work Experience:
- 3 years of experience preferred.
