The Care Coordinator develops trusting connections with clients that foreground respect in order to deliver supportive counseling services using evidence based best practices within their scope of practice.
Accountabilities
- Screening and Assessment: Screens and assesses potential clients to determine the severity of Substance Use Disorder (SUD) and Co-Occurring Mental Health to ensure appropriateness of care. Perform biopsychosocial evaluations to assess client’s needs in treatment.
- Level of Care Placement: Coordinate with medical and clinical team to determine client’s appropriateness for treatment and level of care determination as indicated by the screening and assessment.
- Client Orientation: Performs new client orientation and reviews Participant Handbook, guidelines, program requirements and expectations with client.
- Case Management: Perform case management assessments and help build treatment plans to assist clients with case management needs in conjunction with the peer support team. Present findings and staff in treatment team meetings.
- Treatment Planning: Work closely with client to develop individualized treatment plan and SMART goals integrating the client’s strengths, needs, abilities and preferences
- Care Coordination: Coordinate client care with primary care provider, primary therapist, nursing, Recovery Support Specialists and other outside services as indicated by the client treatment plan.
- Client Education: Inform clients about the various approaches
- Complete accurate, timely clinical documentation consistent with organization standards, rules and regulation
- Understand individualized treatment plans and regularly communicates with the Primary Therapist regarding clinical needs
- Maintain professional standards and follows the treatment policies and procedures of the facility and of the federal, state, and local regulations
- Advocate for the client and the family
- Assist clients in crisis situations involving social, emotional, health, and other problems.
- Promote and adhere to the workplace values of integrity, respect, trust, teamwork, quality, and excellence
Education, Experience, Skills
- Bachelor’s degree in human services or another relevant field required; Master’s degree/MSW preferred
- 2 years of case management and assessment experience strongly preferred
- Experience and training working in the 12 Step Field and/or Dual Diagnosis treatment strongly preferred.
- Must have or, within 30 days from hire, be able to obtain current First Aid and CPR card.
Salary : $50,000 - $60,000
