We are looking a motivated individual to join our team as a Community Liaison/Outreach Coordinator. We are open 365 days a year, 24 hours a day and need team members for all shifts.
The Divided Sky Foundation is an abstinence-based 12 Step residential recovery program located in Ludlow, Vermont, whose mission is to provide supportive and compassionate programming to individuals and their families affected by addiction.
General Description
The Community Liaison/Outreach Coordinator position is responsible for increasing referrals and acting as a liaison between the Divided Sky Foundation and the community and potential clients. The Outreach Coordinator is a self-starter who works independently to fill their schedule with visits to referral sources and developing relationships with medical providers, office staff, and the community.
Accountabilities
- Support program initiatives through outreach activities
- Develops and grows professional relationships with case managers, medical professionals, and other key decision makers of prospective referral sources
- Understands the geographic area for potential referral opportunities
- Assists in the development of marketing/PR materials that will be utilized for building/maintaining relationships for the geographic area
- Assist in development of logistical operations in coordination with admissions team
- Work with network of professionals and follow-up weekly with data and feedback
- Arranging meetings with clients and carrying out facility tours
- Working closely with the Program Director and team to develop new business
- Develop and/or participates in seminars and campaigns for the benefit of obtaining prospective referral resources
- Identify barriers associated with client access to services
- Support admissions team as needed
- Other duties as assigned.

Qualifications and Knowledge
- Ability to work as a member of a team as well as independently.
- Ability to read and interpret documents such as safety rules or policy & procedure manuals.
- Demonstrate proficiency in verbal and written communication skills.
- Ability to apply common sense understanding to carry out instructions in written, oral, or diagram form.
- Ability to deal with problems involving several concrete variables in standardized situations.
- Ability to be flexible in relation to department needs.
Education, Experience, Skills
- BA or BS in a human service field or equivalent work experience preferred
- 1-3 years of experience in human service profession
- SUD and/or MH knowledge preferred.
- Must have or, within 30 days from hire, be able to obtain current First Aid and CPR card
Work Conditions and Physical Requirements
- Work Setting: Indoors and outside work will be required during all seasons.
- Body Positioning: Spend time sitting and standing, making repetitive motions, spend time using hands to handle, control or feel objects, tools or controls. Frequently lift or move up to 10 lbs. Occasionally lift or move up to 50 lbs.
- Communication: Contact with others, electronic mail, in-person discussions, public speaking, telephone, and video.
Salary : $43,800 - $55,400
