What are the responsibilities and job description for the Case Worker position at Ascentria Care Alliance?
The Child and Family Services (CFS) Case Worker provides clinical case management, to families and children in a multi-cultural setting. The Case Worker provides outreach, intensive case management, and direct service in coordination and cooperation with systems including school personnel and educational teams, DCF workers, therapists, lawyers and court personnel, the general public and intra-agency collaterals. The CFS Case Worker is the primary advocate for the child and initiates creative approaches to service delivery and activity that will help the child achieve their potential working toward connection with a permanent resource and long term life plan.

Essential Responsibilities:
- Develops and completes assessments, service and treatment plans, progress notes and any other necessary documentation including court reports or letters to the court in a timely manner.
- Creates a trusting and safe relationship with the client and encourages personal growth of the client
- Provides counseling, coaching and life skills trainings such as PAYA education modules as well as mediation with families
- Advocates for client’s needs as needed.
- Assures that families receive support and supervision for their work with the child.
- Coordinates, conducts and participates in treatment meetings and foster care reviews; and ensures that all community stakeholders are in attendance

Qualifications:
- Bachelor’s Degree in Social Work or a related field required
- One (1) to three (3) year(s) experience working with children and families required
- Bilingual in Spanish and English preferred with the ability to read, write and communicate verbally in English
- Valid Driver’s License, proof of automobile insurance and positive driving record to transport minors in own vehicle if needed required
- Computer skills, including Microsoft Word, and email competence required
- Significant experience working with children with social/emotional/physical problems preferred
- Knowledge of and adherence to federal, state, program and agency regulations preferred
