JOB PURPOSE: Responsible for providing comprehensive case management services to survivors of Domestic Violence, Sexual Assault and Human Trafficking residing in the shelter. This role involves assisting clients identify and access resources, developing safety plans, and navigating the complexities of leaving an abusive relationship. The Case Manager plays a vital role in empowering survivors to rebuild their lives and achieve long-term stability and safety. The Case Manager works under the supervision of the Social Worker.
Job duties/responsibilities:
1. Direct Client Services:
o Conduct intake assessments to evaluate the needs and goals of shelter residents, including safety, housing, legal, medical, and emotional support
o Collaborate with clients to develop individualized service plans that address their immediate needs and long-term objectives
o Provide ongoing support, guidance, and advocacy to empower client to make informed decision about their lives
o Promote a culturally supportive atmosphere in shelter
o Ensure Shelter Standards compliance
2. Resource Referral and Coordination:
o Identify and connect clients with community resources and services, including legal assistance, counseling, housing, healthcare, childcare, and financial assistance
o Advocate on behalf of clients to ensure they have access to the support and resources they need to achieve their goals
o Maintain up-to-date knowledge of available resources and collaborate with community partners to expand access to services for survivors of Domestic Violence, Sexual Assault and Human Trafficking
3. Crisis Intervention and Support:
o Provide crisis intervention and emotional support to clients experiencing distress or facing immediate safety concerns
o Assist clients in developing safety plans to address potential risks and enhance their safety both during their stay in the shelter and after they leave
o Serve as a supportive and nonjudgemental presence for clients as they navigate the challenges of leaving the abusive relationship and rebuilding their lives
4. Documentation and Reporting:
o Maintain accurate and confidential client records, including intake assessments, service plans, progress notes and case management activities
o Ensure compliance with agency policies, procedures and confidentiality standards in all documentation and communication
o Prepare reports and documentation as required by agency policies and procedures, funding, regulatory agencies, and organizational stakeholders
o Accurate and timely documentation of client interactions, service provision, and case management activities in accordance with agency standards and confidentiality requirements
o Enter data into OSNIUM or other related data system
o Maintain data for monthly Direct Services Report (services provided and clients served)
o Answer, provide response and document the crisis line calls
5. Professional Development and Collaboration:
o Participate in regular supervision meetings with the Residential Social Worker to review client cases, discuss challenges and receive guidance and support
o Attending training sessions, workshops and conferences to enhance knowledge and skills in the areas of Domestic Violence, Sexual Assault and Human Trafficking trauma-informed care and case management
o Collaborate with colleagues, community partners, and other stakeholders to promote a coordinated and holistic approach to supporting survivors of Domestic Violence, Sexual Assault and Human Trafficking
6. Community Involvement
o Represent the agency at community events, conferences and meetings to promote the services available at the shelter as assigned by supervisor

Required qualifications, education & experience:
· Bachelor’s degree in human services field.
· Experience working with diverse populations.
· Domestic violence shelter work experience preferred
· Previous experience in case management
· Bilingual or multilingual fluency preferred
· Valid Driver’s license and a privately owned and insured automobile
· Upon hire, completion of required program trainings
· Flexibility to work evenings, weekends and holidays as needed
Working conditions and expectations:
