What are the responsibilities and job description for the Nurse Practitioner position at Asher Community Health Center?
Asher CHC is a Federally Qualified Health Center (FQHC). It is also the only health care provider in Wheeler County. We operate an outpatient primary care clinic in Fossil 5-days a week, and satellite clinics in Spray and Mitchell. Additionally, full dental services are available at all three sites.
In July 2005 ACHC became the first rural Oregon county to receive a state grant to establish a school-based health center (SBHC). The SBHC was established at Mitchell School (K-12, approx. 70 students). The SBHC serves both students and the general community. Previously Mitchell residents had to drive at least 45 miles to Prineville for their Medical Services.

Position Details:
- Proudly serving Wheeler County
- Full scope panel including pediatric, adult, and geriatric patients:
- Develops a plan of care for each patient, including: complete medical history, physical examination, diagnosis, appropriate treatment and/or referrals.
- Review laboratory test results, educate patients as necessary, follows-up as appropriate, initiate patient referrals and tests, etc.
- Take phone calls from patients, pharmacies, etc.
- 1.0 FTE (4-Days a Week)

Qualifications:
- Completion and/or current certification as a Adult Nurse Practitioner by the American Nurses Credentialing Center (ANCC),
American Academy of Nurse Practitioners (AANP)
- Current licensure to practice as a NP issued by the Oregon State Nursing Board.
- Must hold ACLS, BLS, and PALS Certification(s).
