Addiction Services Coordinator

Job Summary
The Addiction Service Coordinator plays a crucial role in facilitating the delivery of services to offenders, ensuring that their needs are met effectively and efficiently. This position requires a compassionate individual who can manage multiple tasks while maintaining a high level of organization and communication. The Service Coordinator will work closely with clients, families, and various service providers to coordinate care plans and support services.

Qualifications:

A bachelors degree in counseling, psychology, social work, or a related field, and be a registered nurse in the State of Ohio or an equivalent combination of education, training and nursing experience. Must possess a valid Ohio driver license with an acceptable driving record.

Responsibilities

• Assess client needs and develop individualized service plans in collaboration with families and relevant stakeholders.
• Coordinate services across various settings, including hospitals, home community care, and outpatient facilities.
• Maintain accurate medical records and documentation in compliance with regulatory standards.
• Provide crisis intervention and behavior management support as needed.
• Facilitate communication between clients, families, and service providers to ensure continuity of care.
• Conduct regular follow-ups to monitor client progress and adjust service plans as necessary.
• Support clients dealing with addiction counseling or other specialized needs by connecting them with appropriate resources.

Skills

• Experience in child family counseling and social work is highly desirable.
• Proficient in behavior management techniques and crisis intervention strategies.
• Excellent organizational skills with the ability to manage multiple cases simultaneously.
• Strong communication skills, both verbal and written, to effectively interact with clients and service providers.
• Knowledge of home community care practices and hospital protocols is beneficial.
• Ability to work collaboratively within a multidisciplinary team environment.
