Job Summary
The Addiction Service Coordinator plays a crucial role in facilitating the delivery of services to offenders, ensuring that their needs are met effectively and efficiently. Qualifications:
A bachelors degree in counseling, psychology, social work, or a related field, and be a registered nurse in the State of Ohio or an equivalent combination of education, training and nursing experience. Must possess a valid Ohio driver license with an acceptable driving record.
Responsibilities
- Assess client needs and develop individualized service plans in collaboration with families and relevant stakeholders.
- Coordinate services across various settings, including hospitals, home community care, and outpatient facilities.
- Maintain accurate medical records and documentation in compliance with regulatory standards.
- Provide crisis intervention and behavior management support as needed.
- Facilitate communication between clients, families, and service providers to ensure continuity of care.
- Conduct regular follow-ups to monitor client progress and adjust service plans as necessary.
- Support clients dealing with addiction counseling or other specialized needs by connecting them with appropriate resources.
Skills
- Experience in child family counseling and social work is highly desirable.
- Proficient in behavior management techniques and crisis intervention strategies.
- Excellent organizational skills with the ability to manage multiple cases simultaneously.
- Strong communication skills, both verbal and written, to effectively interact with clients and service providers.
- Knowledge of home community care practices and hospital protocols is beneficial.
- Ability to work collaboratively within a multidisciplinary team environment.
