Job Summary:
The Value-Based Care Specialist will work to improve patient outcomes and ensure cost-effective healthcare by supporting the implementation, monitoring, and optimization of value-based care initiatives. This role involves collaborating with healthcare providers, payers, and other stakeholders to ensure that healthcare services are aligned with value-based reimbursement models. The Value-Based Care Specialist will also play a key role in tracking and analyzing clinical and financial data to improve quality, reduce unnecessary healthcare costs, and enhance patient care.
Key Responsibilities:
Implementation of value-based care programs to. Collaborate with clinical teams to identify patients who are eligible for value-based care programs. Provide ongoing support to ensure smooth implementation of care initiatives within the clinical practice, including ensuring effective care coordination and follow-up.
Qualifications
Experience working with clinical teams and value-based care models Familiarity with Electronic Health Records (EHR) systems HS/GED
