LOCATION : HOPE Clinic
REPORTS TO : Director of Clinical Services and Quality Improvement
EDUCATION : Graduate of LVN or LPN Program; Current and Clear Vocational or Practical Nursing Licensure.
WORK EXPERIENCE : 3 years nursing experience; One (1) Year Work Experience in Care Management, Quality Management, Case Management or Discharge Planning
SALARY RANGE : $55,000 - $70,000 / year, DOE
FLSA STATUS : Exempt
POSITION TYPE : Full-Time
LANGUAGE : Fluent in English; Bilingual in English and Spanish, Arabic, Burmese, Chinese or other languages is required

JOB SUMMARY :
The LVN, Care Manager supports the delivery of high-quality, patient-centered care by managing and coordinating care for patients with chronic illnesses, complex medical needs, and other healthcare challenges. They work collaboratively with the Director, Clinical Services and Quality Improvements. To ensure patients receive timely, efficient, and appropriate care, addressing barriers to access and promoting health outcomes. The role involves patient education, care planning, monitoring progress, and connecting patients with internal and external resources to optimize their health and well-being. This role is critical in FQHCs, which aim to provide comprehensive, equitable, and accessible health care to underserved populations.
MAJOR DUTIES & RESPONSIBILITIES :
- Facilitates the collaborative interdisciplinary process of care management encompassing assessment, planning, facilitation, care coordination, and evaluation appropriate to the scope of licensure.
- Support Maternal, Population Health and Chronic Care Programs in monitoring, care coordination and data gathering.
- Maintain a paneled caseload with a mixture at risk and high patients and will be responsible for tasks such as care plans and monitoring.
- Document all phone calls and patient interactions in the EHR promptly by utilizing standard clinical documentation guidelines.
- Works closely with Clinical Services, Providers and Program Leads to coordinate care for members.
- Assist the with Quality Improvement Outreach support.
- Schedule follow-up appointments for hospital and ER discharge patients.
- Attend on-site / off-site community engagement activities, clinic events, and / or training as needed;
- Perform other duties as assigned to support HOPE Clinic’s Mission, Vision, and Values.
