Job Summary:
The Care Management Associate (CMA) reports to the CMA Team Leads and works under the guidance of social workers and RN care managers. The CMA supports the senior population by assisting with various care-related tasks, ensuring communication between members, healthcare providers, and other parties to facilitate timely access to necessary services. The CMA plays an essential role in supporting the interdisciplinary healthcare team and performing administrative duties to help maintain smooth care management operations. The CMA is knowledgeable about the needs of the senior population and supports the team in delivering appropriate care.
Essential Duties and Responsibilities:
- Verify member eligibility monthly and as needed, notifying care managers of disenrollments.
- Track and update key health events (medications, flu, pneumonia, diabetes, preventive exams,etc.) in the system.
- Manage authorizations, including creating, updating, and closing them, ensuring timely approvals and maintaining communication with Utilization Management (UM) and providers.
- Ensure accurate and timely updates to the Alert section and contingency plans.
- Assist with vendor communications to confirm service dates and delivery schedules.
- Monitor and update Entity Relationship records to ensure data integrity.
- Assist with address changes for members.
- Support care managers in locating service providers, particularly in difficult-to-serve areas.
- Perform monthly calls and other outreach calls as needed, and assist with obtaining required signatures.
- Maintain accurate and up-to-date records, ensuring that all relevant documentation is linked to thesystem (e.g., MD orders, files in the mail-to-be-linked folder, care plan signatures).
- Complete the monthly reassessment list for care managers.
- Ensure HIPAA compliance and maintain confidentiality in all tasks.
- Follow up on events after UAS/CMCA reassessments, such as sending fall risk letters to PCPsand updating Primary Diagnosis, medications, etc.
- Provide general support to the team by answering phones, forwarding calls, and assisting with service delivery or other non-clinical matters.
- Collaborate within the team environment, demonstrating flexibility in assignments and a commitment to the department's mission and goals.
- Actively participate in care team meetings and communications to ensure smooth and effective care coordination.
- Perform additional duties as assigned by the department.

Pay: $22.00 - $24.00 per hour
