This position is hybrid, with work divided between remote and on-site locations. Approximately 50% of the work will be performed remotely (telecommuting), while the remaining 50% will be conducted on-site or in-person (home visits). These hybrid work expectations are subject to change at the agency’s discretion. The noise level in the on-site work environment is moderate.

- Pay range is between $56,800 to $61,400 annually. Can increase based on experience with possible increases after 6 months from date of hire.
- ACRS pays 100% for employee only HMO/Virtual Plus medical benefits. Additional options are covered anywhere from 75% to 85%.

- Paid Time Off:
- 8 hours of vacation accrued monthly from day 1, available to use after 6 months of employment.

- Employee Assistance Program.
- Screens and assesses client’s ability to function. Verifies, authorizes, and monitors independent provider services.
- Develops, implements, monitors and modifies a written, personalized care plan in collaboration with client/family, physician and others as appropriate. Facilitates implementation of service plans. Makes referrals to Nursing Services RN, as necessary. Participates in reviews of care and discharge planning
- Provides information and discusses options with the client and family regarding client residential care. Upon request, assists with pre-placement family visits and/or with client’s admission process to a nursing home or residential care facility.
- Prepares concise, complete, and accurate program documentation per program and system standards.
- Performs program outreach functions in the community. Explains program policy and services to clients and others.
- Participates in training, meetings, consultations, and conferences.
- Provides team back up when case coverage is needed and assists in the orientation and training of new staff, as requested.
- Education and Experience:
- Master’s degree in social work or social/behavioral science; OR
- Bachelor’s degree in social science with three (3) years experience in social services ; OR
- Bachelor’s degree in other major with five (5) years experience in social services; OR
- AA in social science with seven (7) years experience in social services; OR
- HS diploma with nine (9) years experience in social services.
- Completion of Case Manager training program including State sponsored CORE Training, beneficial.
- Cultural Humility:
- Ability to effectively communicate with individuals with diverse backgrounds and perspectives.
- Ability to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practices. Prefer fluency in Asian and Native Hawaiian/Pacific Islander language. Multilingual and multicultural communication skills are highly valued.
- Knowledge and Skills:
- Knowledge of: the aging process; the aging network service delivery system; other service delivering systems in the community; the purpose of the I&A/CM program and the services it provides; and the responsibilities of I&A/CM staff.
- Ability to: work as a team member with professional and support staff; analyze and draw conclusions from case records; apply casework principles to individual situations; and exercise judgment and organize multiple tasks.
- Technical Skills:
- Effective ability to navigate a personal computer. Competency in use of the Microsoft Office and other relevant software for word processing, calendaring, e-mail, and data entry. Efficiently research and perform various tasks via the internet.
- Additional Requirements:
- Employee must have reliable transportation. If using personal or agency vehicle, required to have valid current Driver’s License, be insurable under agency’s auto policy, and personal vehicle must have current insurance.
