JOB SUMMARY:

With supervision from the Perinatal Program Manager, provides care coordination and case management for assigned perinatal patients, including assessments, information and referral, follow-up, and other supportive services.

Essential Job Functions
  • Provides case coordination/case management for assigned patients.
  • Serves as a guide for each assigned patient through the perinatal program.  
  • Meet each assigned patient at least once each trimester and at least once after delivery and at other necessary appointments as needed.  
  • Completes health education, nutrition, and psychosocial (combined) assessments for initial, trimester reassessments, and postpartum assessments.
  • Provides information, referral, and follow-up for patients.
  • Provides multilingual perinatal education workshops and any other supportive services such as interpretation, as appropriate.
  • It provides health education, adjunct services, or interpretation, as appropriate.
  • Translates/interprets at the medical visit, eligibility, or high-risk visits with support staff, as needed.
  • Notifies Labor Coach Program Coordinator of the need for labor coaching services, and informs patients of the labor coach.  
  • Conducts perinatal patient outreach in the community.
  • Participates in ongoing training to understand various aspects of the Comprehensive Perinatal Services Program (CPSP) program function, including eligibility, registration and scheduling, provision of services, risk assessment and referral, billing, and fiscal issues.
  • Follows perinatal service protocols in the delivery of services.
  • Maintains documentation of services provided.
  • Attends weekly perinatal case conferences and periodic perinatal team meetings.
  • Completes OB Risk Assessment forms for Medi-Cal managed care plans.
  • Assists with scheduling appointments for newborns and postpartum patients.
  • When needed, serves as a bridge of communication between patient and medical provider.  
  • Regularly consults with the Perinatal Program Manager for guidance and accountability.
  • Attends general staff meetings.
  • Serves as the liaison between the Medical Director, Perinatal Program Manager, and Head Nurse in matters relating to patient services, clinic operations, and administrative matters.


General Agency/AHS Duties
  • Fosters an environment that promotes trust and cooperation among clients and staff.
  • Enforces clinic policies and procedures, including maintenance of client confidentiality under HIPAA privacy rules, to ensure that the principles of AHS are implemented.
  • Plays an active role in planning organization-wide activities such as AHS general membership meetings, fundraisers, special clinic days, and other events.
  • Participates in general membership meetings, fundraisers, and other public events, as required.
  • Participates in outreach activities, agency advocacy, and serves on ad hoc committees, as requested.
  • Attends AHS staff retreats and Board of Directors meetings, as required.
  • Participates in agency-wide quality improvement program processes and performance improvement teams to ensure excellence in the quality of services provided across the agency.


Minimum Qualifications
  • A high school diploma with two years’ related experience or an equivalent combination of education and experience
  • Must be at least 18 years of age and be willing to undergo training to understand the perinatal program, and to provide assessment and referral services
  • Bilingual – Abilitiy Cantonese or Vietnamese.


Preferred Qualifications
  • GED required with college courses in health-related science preferred
  • Commitment to working in the Asian community, to community health care with familiarity with community health clinics preferred

  • Ability to work under pressure and in a team environment.
  • Ability to work evenings and/or weekends.
  • Provide patient counseling. 
  • Exercise sound judgment within the scope of authority and practice. 
  • Collaborate with other members of the health care team on an ongoing basis. 
  • Establish and maintain effective working relationships with those contacted in the course of work.
  • Demonstrate appropriate interpersonal and communication skills, both oral and written.
  • Interpret medical terminology between patients and providers preferred.
  • Ability to communicate effectively, get along with coworkers and management, and deal effectively and professionally under pressure.

  • The job requires mobility to work in a clinical setting, strength to lift, carry and position supplies and equipment; touch to conduct patient information, vision to read handwritten and printed materials and examine and observe patients; hearing and speech to converse in person and by telephone.  The job is performed in a moderate-to-loud work environment.
