Aspen Hills Healthcare and Rehabilitation is a sub-acute and long-term care facility located in Pemberton, NJ is seeking a Full-Time CSW/LSW Social Worker in the position of Director of Social Services to join our team of professionals.
Duties include, but are not limited to:
- Admission and Ongoing Assessments/Documentation
- Helping with Adjustment
- MDS 3.0
- Care Planning
- Assisting with Medicaid Applications and other Financial Matters
- Advocacy
- Crisis Intervention
- Discharge Planning
To qualify for this position, the ideal Candidate:
- Must have a NJ Social Work Certification CSW/LSW License in good standing
- Must be able to multi-task and organize
- Must have a passion for the senior population
- Must have strong writing skills
- Must be an effective communicator
- Must be a team player
Familiarity with Point Click Care (PCC) is a plus but not a requirement.
