We are seeking a dedicated and compassionate Home Health Registered Nurse Case Manager to join our team. The ideal candidate will be responsible for providing high-quality nursing care to patients in their homes, coordinating their care plans, and ensuring they receive the necessary services to promote their health and well-being.
Key Responsibilities:
- Conduct comprehensive assessments of patients' health status and needs.
- Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare teams.
- Monitor patients' progress and adjust care plans as necessary.
- Provide direct nursing care, including medication administration, wound care, and health education.
- Coordinate with interdisciplinary teams to ensure continuity of care and effective communication.
- Educate patients and families on disease management, medication adherence, and health promotion.
- Maintain accurate and timely documentation of patient care and progress.
- Ensure compliance with healthcare regulations and standards.
Skills and Qualifications:
- Current Registered Nurse (RN) license in the state of practice.
- Bachelor’s degree in Nursing (BSN) preferred.
- Previous experience in home health, case management, or a related field.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Proficient in electronic health records (EHR) and other healthcare technologies.
- Compassionate and patient-centered approach to care.
- 1 year experience in Home Health preferred
