The Case Manager plays a crucial role in coordinating patient care and ensuring that individuals receive the appropriate services and support throughout their healthcare journey. This position involves working closely with patients, families, and healthcare professionals to assess needs, develop care plans, and facilitate access to necessary resources.
Duties
- Conduct comprehensive assessments of patients' needs and preferences to develop individualized care plans.
- Monitor patient progress and adjust care plans as necessary based on diagnostic evaluations and changing health conditions.
- Maintain accurate medical records and documentation to ensure continuity of care and compliance with regulatory standards.
- Provide education and support to patients and their families regarding treatment options, resources, and self-management strategies.
- Carry a caseload of 12-15 patients MAX a week!
Skills
- MUST HAVE AN ACTIVE RN LICENSE IN THE STATE OF TEXAS
- Experience in clinic environments or emergency medicine is preferred.
- Ability to manage medical records with attention to detail and confidentiality.
- Strong interpersonal skills for effective patient interaction and service delivery.
- Capacity to work collaboratively within multidisciplinary teams while advocating for patient-centered care.
Job Type: Full-time
Pay: From $80,000.00 per year
