Aspen Hospice is looking for an RN case manager to join our team. We need someone who loves to be in a team atmosphere, has amazing work ethic, and enjoys a work/home life balance. Having a heart for hospice and your patients is a requirement. We have a team centered patient driven culture that requires a special kind of person to fit. Duties include case management of home and facility patients including but not limited to medication management, symptom control, DME management, conversations to gain understanding, time discovering what is important to the patient and family, and how we can best serve them. It is a requirement to get to know your patients and provide quality care.
