Shift Differential & Bonus Info :
- SIGN-ON BONUS OF $5000.00 (FULL-TIME HIRES ONLY)
- Evening shift diff is 1.00 and overnight shift diff is 2.00 an hour
- Open to 8-hour and 12-hour shifts

- Provide patient care to residents
- Comply with state and federal regulations.
- Work collaboratively with members of the healthcare team.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team.?
- Valid state license(s)
- Excellent verbal communication skills and strong interpersonal skills
- High integrity and well-grounded work ethic
- Agility and flexibility with changing priorities
- Team-oriented and a practical team-player
- Professional demeanor
- At least one year of experience providing care and services in long-term care (preferred)
