- Participate in HEDIS reporting and quality improvement initiatives.
- Oversee utilization review processes to ensure appropriate care levels are maintained.
- Engage in case management activities, providing support to patients and families throughout their care journey.
- Work closely with hospice care teams to ensure seamless service delivery for end-of-life care.
- Maintain up-to-date knowledge of industry regulations and best practices related to MDS coordination.
Qualifications
