Qualifications:

  • Associate degree in a human services field or related social services experience (Bachelor’s degree preferred).
  • Minimum of one year of experience in a human services role within a hospital, skilled nursing care, or medical facility.
  • Willing to obtain Social Service Designee certification if not already possessed.

Principal Responsibilities:

  • Plan, organize, implement, evaluate, and direct a comprehensive Social Services program.
  • Assist the Social Service Consultant in planning, developing, organizing, implementing, evaluating, and directing facility social service programs. Participate in reviewing and setting policies concerning resident care and quality of life, and in developing facility social work policies.
  • Prepare the Social Services department for the annual survey.
  • Collaborate with Social Services staff, the interdisciplinary team, and administration to promote and protect resident rights and psychological well-being. Prevent and address resident abuse as mandated by law and professional licensure.
  • Complete social histories and psychosocial assessments for each resident to identify their social, emotional, and psychological needs. Complete relevant sections of the Minimum Data Set (MDS) and utilize it to guide care plans.
  • Maintain accurate and timely documentation that complies with federal and state regulations, company policy, and center-specific practices. This includes, but is not limited to, documentation of Advanced Directives, MDS, Social Service History and Assessment, RAPs, Care Plans, Social Service Progress Notes, Behavior Tracking, monitoring cognitive and psychosocial changes, PASARR, and Discharge Planning.
  • Support each resident’s right to self-determination. Document resident refusals of services and inform them of therapeutic alternatives. Involve others as appropriate to educate the resident about available alternatives.
  • Participate in the development of written interdisciplinary care plans for residents, identifying psychosocial needs, goals, and appropriate interventions.
  • Ensure the provision of therapeutic interventions to assist residents with transitions and adjustments to long-term care, addressing their social, emotional, and psychological needs.
  • Provide support and education to residents and their families to assist in understanding placement and facility issues. Refer to appropriate social service agencies when necessary.
  • Coordinate the resident discharge planning process, making referrals for home care services prior to discharge.
  • As part of the interdisciplinary care team, identify discharge teaching needs and communicate estimated discharge dates to the team. Update Point Click Care and make referrals as needed for post-discharge care.
  • Initiate and participate in the completion of the Discharge Transition Plan and Discharge Packet materials. Orient residents and families on the discharge process.
  • Work with the interdisciplinary team and administration to promote and protect resident rights and psychological well-being. Prevent and address resident abuse as required by law and licensure.
  • Collaborate with residents, families, staff, and significant others to foster self-advocacy, improving quality of life and care for residents and the community.
  • Educate residents and families about their legal right to an Advance Directive and ensure that their wishes regarding medical treatment are communicated to the facility, physicians, and healthcare providers.
  • Establish and maintain contact and referral flow with community-based agencies for discharge planning. Maintain an up-to-date list of community resources useful to residents and their families.
  • Participate in quality assurance interdisciplinary team meetings and ensure compliance with company and government requirements for social service documentation.
  • Document progress in meeting residents’ psychosocial needs.
  • Educate nursing personnel on the role of the social worker and the psychosocial needs of residents and their families, including issues related to aging and disability.
  • Educate staff on cultural diversity, resident rights, and abuse prevention.
  • Provide education to residents and families on problem-solving, rights and responsibilities, and community services available to them, particularly for effective discharge planning.
  • Exercise professional judgment in carrying out activities that maximize resident well-being and quality of life.
  • Provide clinical interventions in response to catastrophic events during residents’ stays.
  • Collaborate with healthcare and social service coordinators at other agencies and hospitals to maximize the use of community services.
  • Refer suspected cases of abuse, neglect, or exploitation to Adult Protective Services (APS).
  • Maintain and update a resource file on service providers and vendors. Assist with locating and evaluating service providers, and communicate with them to ensure satisfactory service delivery.
  • Collaborate with the Director of Nursing and Administration to prepare for inspections and surveys. Engage with inspectors, assist in immediate corrections, and reinforce standards cited previously.
