Your Impact
Under the direction of the Core Program Supervisor/Manager, the Lead Care Manager (LCM) provides direct service, as well as coordinates care management and functions as a part of a "Care Team" for the Enhanced Care Management Program (ECM). The LCM oversees specific cases, coordinates health care benefits, provides education and facilitates client access to care in a timely and cost-effective manner. The LCM collaborates and communicates with clients, caregivers/family support persons, and other providers to promote wellness, recovery, independence, resilience, and empowerment, while seeing that access to appropriate services and maximizing client benefit. The LCM also serves as an advocate for clients, an active member of the interdisciplinary team, a liaison with other programs and external health and social service providers in the community.
Your Skills and Duties
- Assess client needs in physical health; mental health; SUD; oral health; trauma; social supports; housing; vocational/employment; wellness; and referral.
- Develop and implement the Individual Care Plan/Health Action Plan.
- Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction Techniques, and Trauma- Informed Care principles when working with clients.
- Coordinate strong communication among service providers, family members and the client.
- Provides services where the client lives.
- Provide physical and mental health activities to client that promote wellbeing inclusive of coaching and skill building.
- Assisting client with finding transportation to and from appointments.
- Provide clients transportation to appointments when indicated.
- Provide care management services including coordination of services, implementation of activities in Client Care Plan, resource, referral and linkage, etc.
- Coordinate/advocate on behalf of client with health care professionals (e.g., PCP) and other service providers..
- Work with client and hospital staff to support Transitional Care plan.
- Accompany clients to critical appointments.
- Evaluate client progress and update client goals.
- Conduct outreach and engagement activities to facilitate enrollment in the ECM program. Outreach and Engagement consists of phone calls, mailed information, events, presentations for other youth and family serving agencies, churches, other areas of worship fields and field visits.
Education and Work Experience
- At least 2 years of relevant work experience
- High school graduate or GED required
- Resident of the community with good knowledge of the resources of this community
- Ability to complete the training program and ongoing educational requirements as assigned
- Ability to work flexible hours, including occasional night/weekend work
PREFERRED QUALIFICATIONS:
- Multilingual capabilities preferred, but not required
- Prior experience as a Community Health Worker, Peer Support Specialist, Support Counselor or similar roles.

Pay Rate: $23.00 - $25.00 Hourly
Salary : $23 - $25
