Nurse Practitioner – Integrative Primary Care & Wellness Center

Position Overview
We are seeking a dedicated and experienced Nurse Practitioner (NP) to join our collaborative clinical team. This position is ideal for a provider with a strong foundation in outpatient primary care and a passion for preventive and integrative healthcare.
As a Nurse Practitioner, you will manage acute and chronic conditions, provide whole-person care, and partner with patients to develop individualized wellness plans that emphasize nutrition, exercise, stress reduction, and mental well-being.
Key Responsibilities
- Conduct comprehensive physical exams and integrative health assessments
- Diagnose and manage a wide range of acute and chronic medical conditions
- Create and implement personalized treatment plans rooted in lifestyle and integrative medicine
- Perform clinical procedures, including venipuncture, IV initiation, and administration of medications and vaccines
- Collaborate with physicians, behavioral health providers, and nutrition professionals to deliver coordinated care
- Educate patients on disease prevention, lifestyle changes, and wellness strategies
- Accurately document patient care in electronic medical records (EMRs)
- Contribute to care continuity and wellness-focused program development
- Engage in wellness initiatives, digital health innovations, and community outreach efforts
Qualifications
- Active and unrestricted Nurse Practitioner license (state-specific)
- Minimum of 1 year of experience in outpatient primary care
- Strong knowledge of and enthusiasm for integrative, lifestyle, and preventive medicine
- Comfort with mental health evaluation, management, and appropriate referral pathways
- Excellent communication and interpersonal skills; ability to build lasting patient relationships
- Strong organizational and multitasking abilities
- Proficiency with electronic medical record systems
- Interest in supporting practice growth through patient engagement and wellness programming
Preferred
- Certification or training in lifestyle medicine, integrative health, or functional medicine
- Experience with digital health tools, remote monitoring, or wellness coaching
