    Work requires the knowledge and skills necessary to provide direct individual services appropriate to adults with mental health diagnoses and / or addictive diseases diagnosis.

    Staff should be able to :

  • Complete all agency and related trainings initially and annually
  • Complete all direct care services with individuals and their families
  • Provide collaborative documentation
  • Keep all credentials and certificates up to date
  • Participate with fellow employees in a respectful and professional manner
  • Abide by productivity standards
  • Follow contract guidelines
  • Demonstrate leadership qualities and characteristics to effectively engage with peers, stakeholders, individuals and families in a professional and collaborative manner
  • ASSIGNED DUTIES
  • DESCRIPTION OF WORK DUTIES AND RESPONSIBILITIES : (Type in or attach current description of duties. Employees are expected to perform their work in a competent and efficient manner. Include % of time).

    Duties Include :

    Ability to provide case management services to individuals with behavioral health diagnosis. Ability to abide by agency and state standards around service delivery and documentation of services. Ability to provide life skill teaching and linkage to valuable resources as well as connecting individuals to natural supports that are in line with individual goals. Employees should complete weekly case management spreadsheets, ensure that other requirements for care are up to date, participate in agency, state and other trainings, participation on stakeholder outreach and collaborative events. Employee should also participate in multidisciplinary meetings, staff meetings, and agency related meetings. Other duties as assigned.
