An exciting opportunity exists as a Client Navigator in our Community Behavioral Health Center! We are looking for a compassionate and professional team member to guide clients through the Same Day Access behavioral healthcare appointment, process, and continued follow-up.
Responsibilities Include
- Manage the client’s visit to Aspire from the intake process, through the visit with the clinician and through the care process
- Respecting the client's choice, explain and collect informed consent for treatment and participating in the grant funded activities, and ensure that clients are fully informed of their legal rights
- Collect and accurately document intake assessments prior to the clinician visit
- Manage several clients each day at different points in their initial visits and treatment plans
- Support client’s non-clinical needs with deep understanding of both Aspire and community resources to remove barriers to the client’s recovery, including addressing social determinants of health
- Participation in Daily Care Huddle to help determine best course of action for each client
- Takes vital signs and weight of client and record in electronic health record

Schedules
Full Time: Monday through Friday 9:30 am – 5:30 pm or 10am - 6pm

- Bachelor's degree Required in a healthcare-related field is preferred
- Knowledge of the health care system and experience or ability to work with busy clinicians to help manage a successful client visit
- Prior experience in researching, and knowledge of community services available for clients
- Prior experience in taking vitals preferred or willing to be trained.
- Community Health Worker (CHW) Certification preferred
